When shopping at participating retailers, you must shop in-store. You can use the Store Finder tool on HorizonExtraBenefits.com or the myTotal Benefits app on your smartphone to look up and locate participating stores in your area.

To shop online, you must visit HorizonExtraBenefits.com or use the myTotal Benefits app to shop the online OTC catalog.

Grocery (online catalog and in-store)

Spend it on Grocery items in participating stores. A limited selection of grocery items are also available online at HorizonExtraBenefits.com or on the myTotal Benefits app.

To purchase items using your Horizon EXTRA Benefits Card all you need to do is:

• Use conveniently at checkout at participating stores.
• To locate participating retailers and view a list of eligible items such as fruits and vegetables, and whole eggs and egg substitutes, visit HorizonExtraBenefits.com or use the myTotal Benefits app.

Utility Benefit

Utilities are the basic services for your home or apartment needs, like water, electric and gas. (Gasoline is not included).

To make payments using your Horizon EXTRA Benefits Card all you need to do is:

• Call your utility provider and use your Horizon EXTRA Benefits card as form of payment.
• Enter the Horizon EXTRA Benefits Card number on your bill or online.

If your service provider does not accept the card, call us at 1-800-480-6598 (TTY 711) or visit HorizonExtraBenefits.com.

If your benefit does not cover the full amount, contact your service provider to submit an alternate payment option. If you have issues, please call Horizon EXTRA Benefits Card Member Services at 1-800-480-6598 (TTY 711) or visit HorizonExtraBenefits.com.

As our valued member, your health is important to us. With Horizon Healthy Journey Rewards, you can earn rewards while taking care of your health. Earn up to $245 per year in rewards for completing a routine health survey and these health screenings:

• Annual wellness visit - $25
• Diabetic eye exam- $25
• Breast cancer screening - $50
• Bone mass density testing - $50
• Colorectal cancer screening - $50
• Health Risk Assessment* - $20
• Diabetic A1C testing - $25

*A health risk assessment is done by calling your Care Manager

To receive your rewards, visit HorizonExtraBenefits.com to fill out and submit a Health Screening Attestation form. You can also download the form and mail it in. For help completing the form, call the Horizon EXTRA Benefits Card Member Services line Monday to Friday, 8 a.m. to 8 p.m. ET, at 1-800-480-6598 (TTY 711).

Once confirmed, your rewards will be automatically loaded onto your Horizon EXTRA Benefits Card. Reward benefits do not expire as long as you are a member of the health plan.
